I'm an idiot! The solution is make sure the monitor you think you should be hearing sound from has built in speakers.

Alienware 17 R4 laptop, Windows 10 pro. The monitor is an LG ultrawide. Audio Manager shows the option for Realtek Speakers and Headhones, LG Ultrawide-8 (Nvidia HD), Nvidia HD Output (unplugged). I've tried all of these without any results. Both Nvidia under output properties say they are HDMI...
