I know this thread is old. But, turns out it was a bad HDD. And, apparently Vista, when booting off CD/DVD loads the files from the DVD to the HDD. I had pulled the HDD to run some diagnostics and discovered that the HDD was failing. After installing a new HDD, everything works...
