3.5mm to headphone/mic adapter

I have a Razer Kraken Pro headset, and the adapter that came with it broke. I've been looking at some 5 1/4 drive bay SD card readers, if someone could post a link here to one that has a headphone/mic combo port, I would be much obliged.

If there's not, a simple adapter will do.
