How could I achieve this audio set up? (Multiple sources to one output, etc.)

I want to be able to have four different audio output sources (Desktop/Game/VoIP/Music), and be able to use a XLR/2.5" microphone input. I'm not necessarily planning on streaming but I'd like to be able to have the flexibility of where my audio goes if I do, and not planning on upgrading any of this hardware in the near future if I do purchase.

I also want to see if implementing a physical EQ for headphones/microphone and an amp for headphones is possible doing this as well. I currently haven't gotten any microphones or headphones yet, but I'd like to know if arrangement is doable.

Not sure about your question per se.

You would use a mixer of some sort.

More specifically you will need a mixer that accepts at least four inputs along with the necessary outputs for your requirements.
