I have a Samsung Galaxy S2 Android 4.1.2 and a Sony Home Theater, connected to WiFi.

So far I have not managed to stream music (let alone video's) from my phone to the home theater system.

Are there any apps available for this?

Many thanks,

What model Sony, is it just a "theatre in a box" or an A/V receiver? If it has an open HDMI port you could get a Google Chromecast and stream to that.

Yes, there is an app for that. It's called "AllShare" and it should be pre-installed on Your Galaxy. It uses a protocol called DLNA (Digital Living Network Alliance.)

Now, to figure out if You have the receiver built into Your "Sony Home Theater"...I don't know...maybe Google knows: https://www.google.com/search?q=sony+dlna

If DLNA is not built into Your existing setup, jonny_street is right, and a ChromeCast Dongle would add a DLNA receiver to an open HDMI port.
