Is my GPU dead?

I notice some graphic glitch when i update my graphic driver but i thought is the windows' scaling issue and i don't bother it. After that, I can't use CUDA to render my Premiere Pro project and now the polygon of games are everywhere. I try to update to newest driver is still the same. Is my graphic card dead?
