Is this Hardware Failure?

I keep getting this. Possible video card failure? Will freeze up my whole computer.
Computer willl often just restart as well.

Graphical artifacts almost always indicate a problem with the card. Update drivers, keep a eye on temps using something like MSI Afterburner. Check the system logs to see if they produce a error message log of what could have happened. Also check to see if fan on card is clean and dust free.

also unlean power will show up in the gpu going nuts. use hardware info watch your 12v rails.
