sd card problem

I have samsung Ace 3 and using 16gb sdcard all the media (pic, videos & audio) in the card i can browse through the file manger .. but can not be read in ANY other app ...
I tried the following :
- formatting the SD
- reset the phone
- update software
- installing media scan apps
...
so i really need a definite solution .. thank you

Does the card work in a computer or another phone/tablet? If the card contents can be seen and opened from the file manager, means the card and file system are good, it may be a device issue.
