sd card wont format or mount

I bought a 16gb sd memory card by Adele. I have used it in the past and worked fine. Now I put it in a new phone a Samsung Illusion and it wont format or mount. It gives the option and I try to format it but it wont work. And I cant save no apps to it cuz it
says it needs to be formated?

plug that sd card on uer computer then try to format. computer can do such things which a smart phone cant!

I don't have a computer just phone its a 16gb evo micro SDHC uhs-1 card I put in phone but can't put anything on it
