I'm not sure what it means when you say a sat is at 91 for example. Is that
the position in the Clark belt? If I'm facing South do the lower numbers
start on my left and increase to the right? So would 90o mean the zenith and
due South? Am I close?

Look at the world today. Is there anything more pitiful? What madness there
is. What blindness. What unintelligent leadership. A scurrying mass of
bewildered humanity, crashing headlong against each other, compelled by an
orgy of greed and brutality. The time must come my friend, when this orgy
will spend itself. When brutality and the lust for power, must perish by its
own sword. -Lost Horizon, 1936

It is the longitude.

The Clark Belt satellites are above the equator. The numbers are
their longitudinal position above the equator.

It is the longitude above the equator. What ever your long is will be
the sat due south of you.
