What's wrong with my SD Card?

I did some digging but I was not able to find any other reports of this issue so I'm hoping someone here can help me.

I have a 64 Gig microSD card that I use in my Galaxy Note II 4.4.2 Kitkat. Recently I started experiencing problems like I'm not able to successfully move any apps to my SD card. After moving an app to the SD card the app becomes corrupted. The same thing happens to any file I try to transfer from the internal memory or from my laptop (via card adapter/ USB chord) to the SD card.

I have tried to format the card using my phone, my sister's phone and my laptop and none of which were successful. I also tried running a chkdsk and scanning for bad sectors neither gave fruitful results. Finally I tried formatting using Disk Management which was also unsuccessful.

All of the data previously stored on the SD card is still there and can be read by any app it seems the issue is writing to the card. Does anyone know what is causing this problem and how I might be able to resolve it?
