Can I have bluetooth speakers set up to stream my smart tv to if the tv is already set to a wired receiver and speakers? I want them for background.

If your receiver has audio monitor out (or headphones connection as last resort), a simple BT transmitter should work.

What if I have an audio out on the smart tv? I have used Bluetooth headphones with that. Would it sync?

Were the headphone in the exact time as the speakers? I would doubt it.
