Often this means something on the disk or operating system is corrupt. Without any more details about how/when this started to happen, I would replace the disk and do a clean Windows setup on the system.

There are many reasons. First try booting in safe mode. It happens that the problem in this case lies in the operating system. Or there are a lot of errors on the hard drive, you can verify this simply by running a check. Another reason may be updating the BIOS, some problem arose during the firmware process.
