It's been said countless times, we can have games blowing people apart with machine guns barely warranting an M rating, yet gasp in horror the second a pair of tits shows up in a game. Simple and effective parental lock:

Use the camera for 360/PS3 and biometrics + voice authorization. If face and voice do not match, no unlock.

It is a parent's responsibility to teach their kids about sex, and starting before the age of 10. Then it is not big deal to them anymore.
