SIM CARD service provider UNLOCK PIN

I am in need of a SIM Service provider unlock PIN for my phone .was wanting to get it unlocked so I can get back to using it through another company.

Only your Service Provider can help you with a PUK or Personal Unlocking Key.

They will only issue the key if all monies on the account are paid up then you can that take key to another Service Provider of your choice.

Is the name of the company printed on the SIM card?
