Creating a customized fillable form for mobile phone use

For reference, I am attempting to create a form that will be used in a healthcare setting. It will have information regarding patient activity, stability, vitals, and grooming. How can I create a fillable form that would mimic our paper form? The intention is for my staff to complete this form from their phone while in the patient's house. They could email it to our office from their phone instead of the hassle they currently go through of faxing physical paper forms.

How can I create a fillable form that would mimic our paper form? The form would just be a PDF, right? Not sure what you are you asking.
