fix a SD card on a phone only

My sister phone have been messing up I format the SD card and 2 or 3 weeks later it's says blank SD card please help me.

Try a different card and see if it works fine. If it does, then the card you were originally using is bad or going bad. If a different one has the same problem, then there is probably a problem with the port which the card goes into.
