How to unblock my sim card when is permanently blocked

Only your Service Provider can remove the block on your SIM and they'll only do that if there are no outstanding and unpaid bills on your account. Call them and ask why it's blocked or go online to your account page on their website.
If they refuse, throw the SIM out and buy a pay-as-you-go SIM card but the risk is the phone is still locked into the original SIM.
