I need to unlock a verizon phone so i can use my corr wireless sim card in it

I have a Verizon Pantech Breaker phone that takes a sim card that I want to be able to put my Corrwireless sim card in and use. When I put it in it says unable to read sim

today i have lost my verizon mobile phone and also forget my emi filter number .how can i get my emi filter number please suggest me.
