I think my phone is being hacked

When i call *#21# mmi code it shows this thing -

call forwarding unconditional.

Is someone listening to my calls ? Or they are being recorded ? (Android)

It could just be that the Service Provider you use doesn't allow that facility because they can't figure out a way of charging you for the facility. Give them a call.
