Well, it depends on how you want to do it. If you are wanting to just move the app from the card to the phone, and it was installed directly to the card (never on the phone) then you can't. It won't move over to the phone. You would then have to uninstall it from the card, download it and reinstall it to the phone itself. This will also mean any data associated with the app will be gone.

In some cases you can save the data prior to uninstalling the app and then re-add the data when you reinstall the app. However that will depend on the apps makers and how they set it up. You would have to check with them.

If it was originally on the phone and you moved it to the SD card, then you can move it back in "Settings" then "Apps".
