Why is windows 7 unable to complete format on my exernal memory card for phone

I have tried and tried to format my external memory card from the phone and my computer keeps telling me unable to format. HELP

One reason could be that your memory card is locked - please check that and push the slider to other side if it is locked - thanks!!!
