My cheating big hacked my phone incall ui is used by his phone I need to know how I can block him out of my phone permanently

Contact your service provider, best thing to do is probably just reset your phone to factory defaults. You can get a new SIM card for your phone with a new number or a new SIM and re-active your old number on it.
