You can... but its not reccomended
You have to make some steps before plug the old SD card into a new phone.
1) Backup your information (via computer). Save your photos, music and files.
2) Format the SD card ( i will advice you to use SD Adapter and card reader ). Normal format is FAT32.
3) Insert the clean (blank) SD card into your new phone.
4) Copy back the information that you want.
DONE!
