Check box control isn't shing in Word 2007

Hello, I have a Microsoft Word 2007. I need to create a form with check box alongside each option. I put the developer tab in the Ribbon, but the check box command isn't showing in the control box. Can you help me find it?I need an active list, not for printing

Click on the little drop down arrow (next to the x that closes the toolbox) in the forms toolbox and then on "Add of Remove Buttons" Find your checkbox and add it to the toolbar. Someone must have removed it.
