Losing Presets on Car Stereo

I just wired up a new car stereo to my Toyota, and checked the source voltage with a VOM. The incoming voltage measured a constant 12V.

However, when I connected the stereo, I noticed the presets won't hold. When I shut the car off, I lose all my settings.

I wonder if a weak car battery can cause this? I was told my battery is going south, but there seems to be enough juice to turn over the engine quite nicely. Should I look into this?

Or, if I accidentally switched the hot source 12v with the ACC yellow wire, would that do it?
I didn't think I did that since the unit turns on fine when I turn to ACC.

Any advice?

Just pay man to do the wiring on radio, i've tried once, took me 6h to wire it, googling wiring, cables and such things, you need car manual and voltmeter as you have (or do it yourself where is the whole fun :D ).
There is 12V(constant) with fuze and there should be another one 12V (switched, it could be red, and also on connector red) which says to radio "hey he turned the key" or known as stand by, i've done it via this site, https://www.crutchfield.com/S-wbWcE56PLYJ/learn/learningcenter/car/car_stereo/installation_guide.html
Colors can be different, but always check manual.
