I have little doubt that a 3rd party seller was involved.
How am I able to post a pic here? I have uploaded a screenshot to imgur and tried posting different links here but says I can't post because of spam.

Did live chat with amazon, they are sending me a replacement with no fuss at all. All I told them that it was already setup when I got it and when I did a factory reset, tracfone wireless came up. Got a feeling this happens quite a bit for them to send a replacement that easy. They didn't ask for pics. Of course, do have to send current one back.

About the topic of needing previous account info when doing a factory reset. Before I send current phone back to amazon, I am going to factory reset again. So is amazon or whoever does these things going to need info from me before they can do anything with the phone? Or will they just need a pin?

Don't think I have ever traded or sold a phone. Always been kind of paranoid when it comes to stuff like this, even after doing a factory reset. I realize the average person will not be able to do anything. And typically, my phones are usually not worth much at all to be worth selling or trading.

When I get the replacement, is there a way to verify that the phone has never been used by someone else?

Upload pictures to a site like imgur.com and then post a link here.

Follow whatever directions Amazon provides.

You'll know about the phone's previous state when you attempt to set it up.

Don't sweat it for now.
I have uploaded pic to imgur and tried all the links using insert link and insert image option here nothing worked. Get a message saying "Your content can not be submitted. Waiting to get phone now, should get it tomorrow.

It is an issue in our end. I'll ask the staff to look into it.
He got the new replacement phone. Going through and setting it up. Turn the phone on and it is already setup just like last phone, didn't have to go through the setup process. Didn't see any signs of any accounts being on there. Before I did anything like add any of his accounts, sim card, connect to wifi, I did a factory reset. Have not connected to old phone at all. Going through the setup process, come to a page that says visible services.

So this is the second phone in a row that has been like this. Is this normal for a new phone to be like this? With the phone supposed to be unlocked, should it show some type of network like visible or tracfone or any other service during setup? Am I just having bad luck with this? I tend to think it shouldn't be like this since when I talked to amazon about last phone, they did a replacement super easy. The phone itself looks super clean, no type of fingerprints on screen or any scratches anywhere on phone. I know fingerprints can easily be cleaned.

When a phone is unlocked, it may still show the previous carrier, but will work with a different carrier's SIM card (otherwise, you would be forced to contact the original carrier to unlock it, if at all). That is just from the software version loaded onto it.

If it is working with your current SIM, you are likely okay. I am surprised though that you didn't get the transfer function from the get go and ask you to register the device, set it up, all of that.

Is the phone able to send/receive calls and texts?
