Well if "Public Shaming" is a standard response to mistakes, it's not surprising that they are not attracting the best and brightest. Of, course, it still beats being sent to Siberia or summarily shot.
 
accidents happen, if my memory serves me right NASA has killed a few people before

You got to wonder what virus was planted on the USB stick. 😀
 
Not only did they fail to launch 3 Satellites into space, but the American's also got 3 used satellites for cheap.
 
"Swimming with the fishes", sounds like the mob had something to do with it... I kid of course!
