Make sure your heatsink is clear of dust build-up. If it is not cooling properly, the fault could be anywhere between the chip and the exhaust. I'd be surprised if there were any thermal paste on the video chip - most of the ones I've seen use a thermal pad. The most likely culprit is dust to build up on the inside of the heatsink (where you can't see it) and create cooling problems.
