You won't get anything very small to run Fallout 4 that can easily mount on the a wall, but you can easily build a small system that will sit near the TV. http/www.tomshardware.com/reviews/newegg-system-builder-marathon-q4-2015-lan-box-pc,4403.html

You likely won't need the extra hard drive storage so could just go with the SSD only, and the video card is also geared towards higher end gamers, that can also be dropped down a notch or two to a 960 or a 950 and get good playable frames.

You will get about 10-25fps (720p) at low settings on that system.
