micro(SD) not detected

My 16GB micro sdhc card not detected in Nokia Lumia 532 & my Windows PC when I go to format won't finish as it states there is no volume when formatting via CMD diskpart. I am able to select the disk but when I CLEAN it, it says "diskpart has encountered an error(cyclic redundancy check) I have a drive letter & says its a removable disk but the status is unusable & I am unable to create a partition primary or new system volume.via disk management or CMD diskpart. It also says every now & then not everytime that Windows can't detect the disk. The SD card is brand new. Please help

What is the brand name of the sd card itself,also where did you purchase the sd card from ?
