First do not post your personal information on a public forum, that is a good way to start getting spam or having your accounts hacked.

Contact your service provider, you can check with your Google account to see if the phone is still broadcasting but from two weeks ago even if someone did not take it and use it with their service the battery would be dead so the phone won't have any signal to track.
