My pc started to lag!

I had this pc 5 years and it was all good but 1 week later it started to lag and only thing it cloud fix it is reinstall...I changed termal paste on cpu and bought 8 gb ram and its still there now CPU is all good gpu is suspicious its good but in dxdiag it only shows intel hd graphics and I runned it on amd like change it on high performance so It could run it on AMD graphics card/7670M.Idk if that problem are drivers or something else but please help!

Help!I had this pc from 2013 and it was all good but next year after every reinstall its good but after some days it starts to bug and bug and over and over again...I dont know what do do.I checked my temperatures cpu is cold as ice gpu same and all other this are ice and nothing is overclocked I tried everything but its doing it again I dont know if its because of drivers or something else I did everything but nothing fixed it so please help me

Check for Hard disk errors and health, it is oldish and if you deleted / installed things much it can lose health.
