Phone and handheld radio audio in one earpiece

I work at a job where I'm always wearing a radio and have to communicate with people over it. I also like to listen to music thru my phone with headphones, which makes hearing the radio hard.

Is there a device where I can have 2 audio streams coming into one set of headphones? My handheld radio has a 3.5mm female out. Also, it'd be nice if the radio audio would mute the audio coming from the phone. Is that too much to ask for?

and maybe add a switch to only connect the radio when you hear it.
