What phone and what SD card. Where abouts did you buy the card? Sometimes you have to be careful with duds.

What make/model of phone are you using? Are you sure it supports 128GB SD cards?

Also, was the SD card formatted on this phone or another one?

Have you tried accessing the SD card from a PC? If so, does it see all 128GB?
