Formatted my sd card, and now it can not be recognized by my phone or my computer

I Formatted my class 10 64gb micro sd hc memory card and now my laptop and phone (S4) can not read my card or even recognize it. Does anyone else have this problem??

What did you use to format it? Was it a cheap Chinese card or name brand? Laptop does not even see that it was inserted as a device?
