how to remove write-protection from micro sdhc card?

I've tried so many times using the disc management in control panel, regedit, cmd, and nothing works. When I try to formatting it on the pc, it says "windows was unable to complete the format"

Write protection is enabled by the person who enables it and a password is set to have access , SD cards do not come with write protection enabled.

Its a SDHC card and i put it in the camera when i bought the camera, an i never had to to set a password so im completely lost on this thing
