Samsung Galaxy prime SD card issues

SD card works on computer, and when inserted into Phone, I get " SD card is blank or as unsupported files" This is the the 3rd Micro SD card I have tried, and have tried them without putting in my computer and then again after. .. and checking formatting, etc. Why doesn't the phone take the cards?

What is the model of the phone? what is the model and size of the sd card?

Galaxy Grand Prime SM-G530W and the card is only 16 gb made by Lexar.

it should take up to 256GB, if it doesn't work on mutliple sd card, it might be the slot on the phone is not functioning
