SD card not inserted

my cellphone is samsung galaxy grand prime plus, I inserted 16gb sd card, after 2 months used(almost 7gb was used). while I am surfing internet my phone was automatically restarting, after that sd card was corrupted. i opened "my files" then it shows "SD card is not inserted" even it is inserted. I tried to open in laptop and PC it can't read also. please help me to solve it. how to recover and fix it.

Sound like the card failed. replace it
