Trying to unlock an SCH-U460 basic verizon phone

I am trying to unlock a basic verizon phone with a model number of sch-u460. I have tried to reactivate it, but I can't enter the information asked without unlocking the phone. I don't know the last four digits of the previous number and I don't know the lock code. If someone could please help, I would be thankful.
