Data recovery from memory card(android)

I am using a 16 GB memory card in my galaxy note 4 for last 2 years.now suddenly i cant see any files saved in card.when i re insert my card it is showing the memory card is unsupported and and it is asking to format my card.but i have some data inside this card .i tried cleaning the card and tried in another phone also but no result.please help..

Try in a computer and run photorec to see if it will find any files on the card.
