See-through black box on screen - what is it?

My Series 2 stand alone connected to a Dish 301 has started putting a
dark gray box covering 90% of the screen over >some< of the
recordings. It may be related to reruns, as it seems as though it
mostly happens while recording those. Does anyone have a solution?
You can watch the recording, but it's a major PITA. I have had this
with both the a and b versions with release 240.

Check the Closed Captioning settings on your TV.

I had checked for that, closed captioning is not set. If it was, my
TV just superimposes the words anyway, not a box covering the entire
screen. Recordings made on the Dish DVR don't have the problem,
either. It's either the TIVO or the Dish "dumb box" 301 causing the
problem, not the TV. It just started fairly recently, say a month
ago.

CC has various iterations and many TVs are capable of viewing all/many
of them. My TV does as you suggest HOWEVER there are additional settings
that can also create the black box you describe. I can turn CC on and
off via one menu but choose the type via another. While your
investigation does seem to point to other possibilities, I'd suggest
having another look.

The clincher to me is that it only does it to some shows and always to
those shows. It's as though it got recorded with the box. The same
show recorded on my Dish 508 at the same time looks fine on the same
TV. Other shows are fine. Also, the second tuner on my TV doesn't
have captioning, but does show the box.
