I'm looking for a receiver or a splitter that uses HDMI audio+video in and has both optic audio, 3.5mm surround, and HDMI video out. The price doesn't really matter, but I'd like to keep it barebones and cheap - without sacrificing quality anyway.

Solution
It sounds like you want an HDMI switch with optical audio and 5.1 analog out? I have only seen them with 2.0 analog audio out.
You might need to use a separate HDMI switcher with digital audio out and an 5.1 decoder.

Most surround receivers don't have preamp outputs so if you go for one make sure that it has them.

Thank you! That's exactly what I'm looking for, well except the double device deal - but that's trivial. Thank you again. I may leave the 5.1 analog out and just use optical for simplicity and ease of management.

Just one question about a switch like this: If the monitor or TV has audio capability, will it disable the audio on the output device and only play on the audio out(s), vise versa, or both?
